Creatine and Hydration: Do You Really Need to Drink More?

"Drink 4–5 litres a day while loading" is repeated everywhere. It doesn't appear in either of the two major clinical sources on creatine. And what those sources do say points in the opposite direction.
Start creatine and you'll be told to drink a lot more water. The number is usually four to five litres a day during loading, stated with the confidence of a clinical guideline.
So we went looking for the guideline. It isn't there.
The International Society of Sports Nutrition's position stand on creatine (the closest thing this field has to an authoritative consensus document) contains no litres-per-day hydration recommendation at all. And its actual findings on dehydration and cramping run directly counter to the warning the number is attached to.
The Short Answer
Bottom line: creatine pulls water into your muscle cells rather than out of your body, the major position stand reports no increase in dehydration or cramping among users (in fact less of both) and the widely quoted 4–5 litre loading target has no clinical source behind it.
Four common claims, against what the position stand actually says:

The through-line: the water isn't leaving your body, it's moving into your muscle cells. So there's no deficit for a bigger water bottle to fill.
Where the 4–5 Litre Number Comes From
As far as we can establish: supplement marketing. It doesn't trace to the clinical literature.
We went to the ISSN position stand on creatine (Kreider et al., 2017), a document that runs through dosing protocols, loading phases, safety across populations from infants to the elderly, and long-term use up to 30 g/day for five years.
No litres-per-day fluid target appears anywhere in it.
That's a meaningful absence. This is a document that specifies creatine dosing precisely. If a specific hydration protocol were an established requirement of taking creatine, this is exactly where it would be stated.
What appears to have happened is a plausible-sounding inference (creatine causes water retention, therefore you must need lots more water) that hardened into a specific number through repetition across brand blogs and forum posts, and now gets quoted as though it came from a lab.
Why the inference is wrong is the interesting part, and it's the next section.
What the Water Actually Does
It moves into your muscle cells. It doesn't leave your body, which is why it doesn't create a deficit to replace.
The position stand quantifies it directly:
"creatine loading promoted a short-term fluid retention (e.g., about 0.5 – 1.0 L) that was generally proportional to the acute weight gain observed"
Half a litre to a litre, and it tracks the scale weight change, which is why the first week on creatine typically shows a 1–2 kg gain that isn't fat.
The critical detail is where it goes. The document is specific:
"creatine supplementation increased intracellular water and reduced thermoregulatory and cardiovascular responses to prolonged exercise"
Intracellular. Inside the cells, not pooled under the skin. This is why the common description of creatine causing "bloating" or a "puffy" look misreads the mechanism: the volume increase is happening within muscle tissue.
And note the second half of that sentence, which almost never gets quoted: increased intracellular water reduced thermoregulatory and cardiovascular strain during prolonged exercise. Extra water inside muscle cells is a buffer, not a liability.
So the logic behind the drink-more advice inverts. Water held inside your cells isn't water you've lost. There's no deficit being created that needs topping up beyond your normal intake.
Does Creatine Actually Dehydrate You?
No. And the position stand addresses this claim head-on, because it's been circulating for decades.

The relevant passage is unusually blunt for a consensus document:
"the peer-reviewed literature demonstrates that there is no evidence that: 1) creatine supplementation increases the anecdotally reported incidence of musculoskeletal injuries, dehydration, muscle cramping, gastrointestinal upset, renal dysfunction, etc."
Note "anecdotally reported." The authors are naming these as claims that circulated without evidence and were then tested. And didn't hold up.
The comparative data goes further than neutral:
"creatine users experienced less incidence of cramping, heat illness/dehydration, muscle tightness, muscle strains/pulls, non-contact injuries, and total injuries"
Fewer heat-illness and dehydration incidents among creatine users than non-users. The authors go on to suggest creatine may actively reduce heat-related illness risk when used as a hyper-hydration strategy, with the extra intracellular water functioning as a reserve during heat stress.
That's a complete reversal of the folk warning. The supplement widely believed to dehydrate athletes is associated in this literature with fewer dehydration events.
What About Cramping?
Same answer, same source: creatine users cramped less, not more.
The cramping myth is durable because it has a satisfying mechanism attached: creatine pulls water into muscle, therefore less water elsewhere, therefore cramps. It sounds like physiology.
The measured data doesn't support it, per the passage above. Creatine users reported less cramping and muscle tightness.
If you cramp during training on creatine, the likely causes are the ordinary ones: heavy sweating without sodium replacement, unaccustomed training volume, fatigue. Those are worth addressing on their own terms. What to Actually Do
Drink normally for your body size and training load. The creatine doesn't change the equation.

- Use your ordinary target. Our hydration calculator sets a figure from weight, activity and climate. That figure doesn't need a creatine adjustment on top.
- Expect 1–2 kg on the scale in week one, and don't interpret it. It's intracellular water, proportional to roughly 0.5–1.0 L of retention. It isn't fat, and it isn't a sign you're under-hydrated.
- Skip the loading phase if it bothers your stomach. GI upset during loading is the one commonly-reported complaint. A steady lower daily dose reaches the same muscle saturation more slowly, without the bolus.
- Don't force-drink to hit an invented number. Deliberately drinking four to five litres because a blog said so carries its own risk. Diluting blood sodium is a real hazard in athletes, and it's a worse one than the problem you're trying to prevent.
- Take electrolytes for your sweating, not for your creatine. If you train hard in heat, sodium replacement matters. But it matters identically whether or not you're supplementing creatine.
That last point is why there's no product recommendation on this page. The evidence here says creatine doesn't create an additional fluid or electrolyte requirement, and selling you an electrolyte product on a page that just established that would contradict it. FAQ
Do you need to drink 4–5 litres of water a day on creatine? Does creatine cause cramping? The research points the other way. The ISSN position stand reports that creatine users experienced less incidence of cramping, heat illness and dehydration, muscle tightness, strains and total injuries than non-users: the opposite of the common gym-floor warning.
Do you need extra electrolytes on creatine? Not because of the creatine. The water creatine draws in moves into muscle cells rather than being lost, so there's no additional electrolyte loss to replace. If you're training hard in heat you may need electrolytes for the sweating. But that's the training, not the supplement.
