Hot Weather and Heat-Wave Hydration

Your body adapts to heat faster than you would expect, and in two stages, which changes what you need and when. It also means the warning sign most people are taught for spotting heat stroke is the wrong one.
Every heat-wave article says drink more water. Fine, but it leaves out the two things that actually change your decisions.
The first is that your body adapts within days, and it adapts in two separate stages that arrive at different times. That changes what you need, and when.
The second is that the rule most people carry for spotting heat stroke (hot dry skin means emergency, sweating means you're fine) is not reliable. Believing it can cost someone time they don't have.

The Short Answer
Bottom line: heat acclimation makes you sweat more while losing less sodium, so your electrolyte needs are highest in the first few days of heat and fall as you adapt. And the reliable sign of heat stroke is confusion, not dry skin.
Three things worth carrying:
- Adaptation is fast and staged. Salt conservation starts around day 3. Increased sweating starts around day 7 or 8.
- The unacclimatised person needs electrolytes most. Sweat sodium drops to roughly 60% of baseline within 10 days.
- Confusion is the emergency signal. Heat stroke can present in someone still sweating heavily.
What Heat Actually Does to Fluid Loss
Sweat, overwhelmingly. And unlike the cold, it gives you honest feedback.
Heat drives fluid loss through evaporative cooling. You sweat, it evaporates, and you lose heat along with the water. The hotter it is and the harder you work, the more of both.
This is the mechanistically simple case, and it's worth contrasting with the opposite season. In winter, four separate mechanisms stack: suppressed thirst, doubled respiratory loss, cold-induced diuresis, and sweat hidden under layers. None of them announces itself. Heat mostly gives you one big obvious loss route plus a working thirst signal.
Which means the heat problem isn't usually noticing. It's that the losses are large, and that what's in the sweat changes as the season goes on.
You Adapt Faster Than You Think
Within about ten days your sweating changes substantially, and the two adaptations don't arrive together.

A heat acclimation study put participants through 10 consecutive days of cycling at 33°C and 65% relative humidity, measuring sweat rate and composition at the arm and back.

Two adaptations pull in useful directions at once. You cool yourself more efficiently, and you hold onto more salt while doing it. Chloride tracked sodium; potassium stayed roughly stable.
The timing detail is the part nobody publishes. The two changes appeared on different schedules: sodium conservation from day 3, but sweat-rate increases only from day 8 at the arm and day 7 at the back.
So your body protects your salt first and improves your cooling second. In the first few days of a heat wave or a hot-climate trip you are in the worst configuration, not yet sweating efficiently and still losing sodium at the unadapted concentration. That's the window where heat feels hardest, and there's a physiological reason for it.
One caveat, stated because the sample is small: this was 8 participants. The direction and timing of these adaptations are well established across the heat-acclimation literature, but treat the exact percentages as indicative rather than as numbers to plan around.
Which Means Your Electrolyte Needs Go Down
This inverts how the category is marketed. The person who most needs electrolytes in summer is the one who just arrived in the heat, not the one who's been in it for a month.

Follow the arithmetic. If sweat sodium falls to around 60% of its starting concentration over ten days, then someone doing identical work in identical conditions is losing meaningfully less salt per litre in week two than in week one.
Practically:
- First days of a heat wave, a hot-climate trip, or the start of summer training. Highest sodium loss per litre, and you haven't adjusted your habits yet.
- Two weeks in. Sweating more, losing less salt per litre, generally coping better.
- Sedentary indoors with air conditioning. You're not really acclimating, and you're not losing much either.
A higher-sodium electrolyte drink mix built for performance and recovery. Its official product listing states 1,120mg of sodium per stick, noticeably higher than a standard mix.
Scoped to the unacclimatised window specifically: the first days of sustained heat exposure or outdoor exertion, when sweat sodium concentration is still at its unadapted level. The honest part is that this need has an expiry date. As you acclimate over one to two weeks, sweat sodium falls substantially and the case for a high-sodium mix weakens accordingly. It is not a daily summer requirement, and it does not make drinking more water safer. Work out your own sweat rate rather than assuming, and check the current label since formulas change.

One boundary matters here. Sodium replaces what sweat removes. It does not license drinking more. Electrolytes and volume are separate decisions.

Heat Exhaustion vs Heat Stroke
The widely-taught skin test is unreliable. The reliable signal is neurological.
Most people are taught that sweating means heat exhaustion and hot dry skin means heat stroke. It's memorable, and it's not dependable.
OSHA's own listing of heat stroke signs is: "Confusion, Slurred speech, Unconsciousness, Seizures, Heavy sweating or hot, dry skin, Very high body temperature, Rapid heart rate."
Read that fifth item again. Heavy sweating or hot, dry skin. Exertional heat stroke, the kind that happens to athletes and outdoor workers rather than to frail people in an un-airconditioned flat, frequently presents in someone still visibly sweating. Waiting for dry skin before escalating means waiting through the window where cooling helps most.

The discriminator is the mental state, not the skin. Someone confused, slurring, or not making sense in the heat is a medical emergency regardless of how wet their shirt is.
OSHA's operational rule is the one to memorise: "When in doubt, cool the worker and call 911." For heat stroke the instruction is to "call 911 immediately and cool the worker with ice or cold water until help arrives." Cooling and emergency services, in that order, ahead of anything else.
Note what isn't on that list: giving them a drink. Someone with altered mental status can't safely swallow, and cooling is the intervention that changes the outcome.
The Other Thing It Might Be
Heat exhaustion and exercise-associated hyponatremia look alike during endurance events, and the fluid response differs.
Nausea, dizziness, headache, fatigue and weakness appear in both lists. If someone has been drinking heavily through a long hot event, overhydration is genuinely on the table alongside heat illness. The clinical guidance for hyponatremia specifically flags distinguishing it from heat exhaustion.
The practical tell is the same one that runs through both topics: weight change. Down suggests fluid loss. Up or unchanged after hours of exertion suggests the opposite problem. When someone is confused, don't spend time working out which it is. Cool them and call emergency services. That's the correct response to heat stroke, and it does no harm while you wait either way.
What To Actually Do in a Heat Wave
- Drink to thirst, and check it's working. Unlike the cold, heat generally leaves thirst reliable. But it weakens with age, so don't rely on it for older relatives.
- Front-load before going out, rather than trying to catch up in the heat.
- Take electrolytes seriously in the first week, less so after.
- Weigh yourself around one hot session to replace estimates with your number.
- Stop on dizziness or feeling unwell. Pushing through is how heat exhaustion becomes heat stroke.
- Check on people who won't call you. Older adults living alone are the highest-risk group in a heat wave, and dehydration in that setting often goes unnoticed until it's an emergency.
- Know the escalation line in advance. Confusion means emergency services, not a rest and a drink.
Your daily target still starts from the ordinary calculation. The hydration calculator includes a heat adjustment. What heat changes is the size of the losses and the cost of ignoring them.

How long does it take to get used to the heat? Days rather than weeks, and the adaptations arrive on separate schedules. Does dry skin mean heat stroke? Not reliably, and using it as the test is dangerous. OSHA's own listing includes heavy sweating or hot dry skin among heat stroke signs, because exertional heat stroke often presents in someone who is still sweating heavily. Waiting for dry skin costs time you may not have.
Should you give someone with heat stroke water? Cooling and emergency services come first. OSHA's instruction is to call 911 immediately and cool the person with ice or cold water until help arrives, with the operational rule: when in doubt, cool the worker and call 911. Someone with altered mental status also cannot safely swallow.
