So we went and read the actual labels. All three of them.
What's there is genuinely worth knowing — but it's close to the opposite of the story being told about it. The warning is old, not new. The mechanism everyone leads with (the drug switches off your thirst) is the smaller of the two risks. And the bigger one is sitting in plain sight in the adverse-events table.

The Short Version
Bottom line: the dehydration risk on a GLP-1 isn't mainly that you forget to drink — it's that vomiting and diarrhea take fluid out faster than a reduced appetite lets you put it back, and that risk scales with your dose.
The three things worth taking away:
- The kidney warning is not a new discovery. It has been on the semaglutide label since Ozempic's original 2017 approval. The 2026 dates in the headlines are revision dates on the current label, not the date a warning appeared.
- The thirst effect is real but modest. The best human evidence shows roughly a one-cup reduction in fluid intake across eight hours — and it was measured using a different GLP-1 drug than the ones most people take.

Is the Kidney Warning on Ozempic and Wegovy New?
No. The acute kidney injury warning tied to dehydration has been on the semaglutide label since the drug's original 2017 approval, and was strengthened in 2022.
Here's the actual text, from the current Ozempic prescribing information (Section 5.6, revision date 5/2026):
"There have been postmarketing reports of acute kidney injury, in some cases requiring hemodialysis, in patients treated with semaglutide. The majority of the reported events occurred in patients who experienced gastrointestinal reactions leading to dehydration such as nausea, vomiting, or diarrhea. Monitor renal function in patients reporting adverse reactions to OZEMPIC that could lead to volume depletion, especially during dosage initiation and escalation of OZEMPIC."
The Wegovy label carries near-identical language under the heading "Acute Kidney Injury Due to Volume Depletion" (Section 5.5, revision date 6/2026). Zepbound — a different molecule, tirzepatide — has the same heading at Section 5.3, revised 4/2026, telling prescribers to monitor kidney function in patients whose side effects could lead to volume depletion.
Three different labels. Three different 2026 revision dates. One warning that predates all of them.
Why this matters practically: a warning that's been stable for nine years is a known, characterized risk with a known mechanism — not an emerging signal that should make you reconsider your prescription. Acute kidney injury here means a sudden drop in kidney function, and in this context it's a downstream consequence of losing too much fluid, which is exactly why the label's instruction is to monitor rather than to stop.
The detail worth carrying forward is the when. The Ozempic label singles out "dosage initiation and escalation" — the weeks you step up to a new dose. That's the window where this actually applies to you.
Do GLP-1 Medications Actually Suppress Your Thirst?
Partly — and by less than nearly every article on this topic implies. The strongest human evidence shows a reduction of about one cup of fluid over eight hours, measured with a GLP-1 drug that most readers aren't taking.
This is where the coverage gets ahead of the science, so it's worth walking through what's actually been shown.
The biology is real. GLP-1 receptors are concentrated in thirst-inhibiting neurons in a brain region called the lamina terminalis — the area that handles water balance and tells you when you've had enough to drink. So there is a genuine anatomical basis for expecting these drugs to touch thirst, not just appetite.
The human evidence is thinner than the biology suggests. Two published trials have actually measured it, both using dulaglutide (sold as Trulicity):
- In 34 patients with primary polydipsia — a disorder of compulsive excessive drinking — three weeks of dulaglutide cut fluid intake during an eight-hour visit from 2,950 mL to 2,460 mL. That's a 490 mL reduction, and it was statistically solid (95% CI −780 to −199, P = 0.002).
- In 20 healthy volunteers, the same drug reduced eight-hour fluid intake by 250 mL (P = .002) and suppressed copeptin — a marker for the body's water-retention hormone — by about 12%.
That second study is the one that matters most for you, because healthy volunteers are closer to an ordinary GLP-1 user than someone with a compulsive-drinking disorder. And the number it produced is 250 mL across eight hours. Roughly one cup.
Three caveats keep this honest, and none of the competing articles mention them:
- Both trials used dulaglutide, not semaglutide or tirzepatide. The trial authors specifically note that molecule size affects how much of a GLP-1 drug reaches the brain, so the effect on Ozempic, Wegovy, or Zepbound has not been directly measured.
- Both ran three weeks. Nobody has published what happens at six months.
- The polydipsia trial's own brain-imaging substudy failed to confirm the mechanism. The behavior changed; the scan didn't show why.
So: if you feel less inclined to drink since starting, that's plausible and you're not imagining it. But a one-cup drift is something a deliberate routine fixes easily — and it is not, on its own, what puts someone in the kidney-injury reports.
So What Is Actually Dehydrating You?
Gastrointestinal fluid loss — and unlike the thirst effect, it scales steeply with your dose. This is what the labels themselves blame for the kidney-injury cases.

Read down the vomiting column. It goes from 5% on the lowest Ozempic dose to 24% on Wegovy's 2.4 mg — roughly one in four people. That's the same molecule as Ozempic, just dosed for weight loss instead of blood sugar.
This is the single most useful thing on the page, so it's worth stating plainly: the hydration advice that fits a person on 0.5 mg of Ozempic is not the advice that fits a person on 2.4 mg of Wegovy. Most articles on this topic write one set of instructions for everyone taking "a GLP-1." The label data says those are meaningfully different situations.
It also reframes the whole problem. Forgetting to drink is a scheduling issue, and a schedule fixes it. Losing fluid through repeated vomiting or diarrhea is a losses issue — and you can't out-sip it with plain water alone, because you're losing sodium and potassium along with the water.
How Much Should You Actually Be Drinking?
There is no established GLP-1-specific daily fluid target — and anyone giving you one is making it up. Your starting number is the ordinary adult calculation; what changes on a GLP-1 is the delivery method, not the target.

Two rules make the difference between this working and not:

What Actually Helps When Nausea Makes Drinking Hard
